FULL FEES
$275.00 for Evaluation, $200.00 for Psychotherapy, $150.00 for Medication
INSURANCE
The practice is currently contracted with the following carriers:
AETNA, UBH/USBH/Pacificare, Healthnet/MHN
BLUE CROSS will no longer be accepted for new patients as of 2012. Existing patients with Blue Cross will be continued.
BLUE SHIELD will sometimes be covered - in most cases this will be coverred by United Behavioral Health or US Behavioral Health at 100% - otherwise - patient's will be required to pay at time of service for full fee, but will then be presented with a superbill which they can send to their insurance for a 50-90% reimbursement.
CIGNA will no longer be accepted. New or existing patients with Cigna will have to pay the full fee in the office, and the submit our invoice to be reembursed later by their insurance carrier.
If the practice is contracted with the patient’s insurance panel, then the insurance can be billed. The patient will be responsible for the co-pay as arranged by the insurance company, and the practice will be responsible for submitting the balance of the bill. The patient will be responsible for communicating with the insurance if there are difficulties.
NOTE: If the insurance cannot be billed, the patient will be responsible for the full fee.
