Billing and Fees

FULL FEES

$275.00 -- Initial Evaluation
(45-60 minutes)  

$200.00 -- Psychotherapy
(45-50 minutes)

$150.00 -- Medication
(20-30 minutes)

DISCOUNTS

Dr. Hersevoort regularly maintains a fixed number of discounts available at up to the 50% level.  These discounts are need based and are subject to change. 

Discounts for fees will be arranged in advance of treatment.  Discounts will expire each January and July and will need to be reevaluated at that time.

NOTE: Discounts must be arranged with Dr. Hersevoort directly, and cannot be arranged through Community Psychiatry.

INSURANCE

The practice is currently contracted with the following carriers:

AETNA, BLUE CROSS,  UBH/USBH/Pacificare, Healthnet/MHN

CIGNA (will be partially covered - patient's will be required to pay at time of service for full fee, but will then be presented with a superbill which they can send to their insurance for a 50-90% reimbursement). 

BLUE SHIELD (will usually be covered - in most cases this will be coverred by United Behavioral Health or US Behavioral Health at 100% - otherwise - patient's will be required to pay at time of service for full fee, but will then be presented with a superbill which they can send to their insurance for a 50-90% reimbursement)

If the practice is contracted with the patient’s insurance panel, then the insurance can be billed. The patient will be responsible for the co-pay as arranged by the insurance company, and the practice will be responsible for submitting the balance of the bill.  The patient will be responsible for communicating with the insurance if there are difficulties.

In order to have insurance pay for your visit, you MUST contact them prior to the first appointment.  The insurance must be called by the patient, and the registration information must be for "outpatient mental health."  The information on the card is often insufficient as it relates to medical care which is separate from mental health care.  You must fill out the registration and insurance information completely.

NOTE: If the insurance cannot be billed,  the patient will be responsible for the full fee.
